Let's All Do Our Part to Keep On-Campus Learning Possible Students or employees traveling outside of Miami-Dade County need to submit a travel form before leaving on your trip. This includes travel within Florida, in the United States and International travel.
_______ Travel by car outside of Miami-Dade County: This includes social gatherings with persons outside my household, visit to any theme park, etc. Must Self-Isolate for 5 work/school days upon return to Miami and monitor for symptoms. Individual must test for COVID-19 if symptoms develop. _______ Travel by Plane within the US and Florida: Must isolate for 10 days upon return to Miami and test using PCR test (not antigen). Negative test result must be presented to Highpoint Academy to return to school or work. _______ International Travel by Plane to a foreign country: Must isolate for 14 days upon return to Miami and test using PCR test (not antigen). Negative test result must be presented to Highpoint Academy to return to school or work. |

Parent/Legal Guardian must sign a COVID-19 Liability Waiver affirming to adhere to safety policies and guidelines established by Highpoint Academy. Students without a signed waiver will not be allowed to return to school.
By signing this Agreement, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you and your child(ren) may be exposed to or infected by COVID-19 by attending Highpoint Academy.
While Highpoint Academy cannot guarantee that you or your child(ren) will not become infected with COVID-19, we're taking every possible measure to provide our students with a safe learning environment.
